Sorry for the delay. I finally found an answer for you. I am aware of two businesses that rent surf boards:

I found both at Santa Barbara Beach, located near Ribeira Grand.

Bom dia,
I would like to know if there is any surfshop in Sao Miguel where it be possible to rent a shortoard. I've been searching in internet but I did not find any.
I sent an e-mail and I called to the following contacts but I had no answer:
Black sands & Azores surf co.
Please, give me a contact o tell me if it is possible to rent or not and, if you know, sizes and prices. I need an answer as soon as possible because today I have to confirm to the air company if I travel with my stuff or not.
